Upon arrival, please report to the reception desk. On your initial office visit, you will be asked to fill out a patient information form before you see the doctor. You are responsible for bringing any referrals and making any co-payments required by your insurance carrier at the time of your visit.
Please bring the following to your appointment:
1. Your insurance card
2. Driver's license or other government issued identification
3. Physician referral forms if required by your health insurance plan. We will not be able to treat you without a current referral form.
4. A list of all of your current medications, both prescription, herbal and over-the-counter, including dosage and frequency of each. Please list these on the Medical History Questionnaire (see below)
5. Pertinent information about your current and past medical and surgical history. This should also be detailed on the Medical History Questionnaire (see below)
6. Any recent chest x-rays, CT scans (this means the actual films or CD copy as well as the reports), or other records you may have from your regular physician

At the conclusion of your visit, you will be given the total charges for that day. We will also inform you of any outstanding charges for hospital care if you were treated in the hospital by a member of our group.
In an effort to reduce the cost of care, we request payment at the time of service. At the present time, our practice participates with the following health insurance plans:

Please remember that insurance is merely a way to reimburse the patient for doctor's fees and is not a substitute for payment. If you have any questions regarding our services or fees, please feel free to discuss them with us and, if you need further assistance, you will be directed to our billing department. If unusual circumstances necessitate special arrangements for payment of our bill, please bring this to our attention. We will be happy to work with you.

Our office hours to call to schedule an appointment is Monday - Friday from 9:30 am - 4:00 pm. Should you find it necessary to cancel or change an appointment, please notify us as early as possible.
Occasionally, the doctors' schedules and hospital emergencies necessitate a change in your appointment. When this occurs, we will do our best to contact you to avoid an extended wait or unnecessary trip on your part.

During regular office hours our staff will notify the appropriate doctor and she will return your call. When the offices are closed, dialing the office phone will allow you to leave a message or you will automatically be forwarded to our answering service, through which you may contact the covering physician. In an emergency, call 911 or go to the nearest hospital emergency room. The staff there will contact the physician on call.

All prescriptions and renewals should be requested during regular office hours. Please have the name and number of your pharmacy and the name and dose of medication handy when you call for a renewal.
